Adding Laakarilehti.js

• An English introduction to Finnish Medical Journal: https://www.laakarilehti.fi/english/

The following are left for the end user after translation

• Jounal article page numbers for those with shortend URL /www\.laakarilehti\.fi\/e\d+/
• Securing PDF file if no subscription access at runtime.
  • Applies also to downloading sll in Duodecim.js.
• Translation via subscription access: whether shortened URL leads to publicly available full text.
  • When on proxy, requestDocument() always uses the proxied domain. See directAccess().

• Revised (supplementary) PDF link filtering reducing unnecessary links.

  • onCampus() name was scrapped for directAccess() that also verifies PDF link validity.
    • The directAccess() variant in Laakarilehti.js still uses requestDocument().
